- It is used after a stroke or mini-stroke, for a neck bruit and before major surgery.
- Narrowing is graded as a percentage; severe narrowing may need specialist referral.

What is a carotid Doppler?
The two carotid arteries run up each side of the neck and carry most of the blood to the brain. A carotid Doppler (carotid duplex) uses ultrasound to show the artery walls and any fatty plaque, and Doppler to measure the speed of blood flow. Faster flow through a segment means narrowing, which is graded as a percentage. The vertebral arteries at the back of the neck are checked for the direction of flow, and the wall thickness (intima-media thickness) may be measured.
Why do doctors order a carotid Doppler?
- After a stroke or a transient ischaemic attack (TIA, mini-stroke), to look for a source in the neck arteries.
- Sudden temporary loss of vision in one eye, weakness, numbness or difficulty speaking.
- A bruit (whooshing sound) heard over the neck.
- Before heart or major vascular surgery.
- Risk assessment in people with diabetes, high blood pressure, high cholesterol or smoking, when the doctor advises it.
- Follow-up of known narrowing or after carotid surgery or stenting.
If symptoms of stroke are happening now (face drooping, arm weakness, slurred speech), go to the nearest emergency department immediately; do not wait for a Doppler.

Understanding your results
The report describes plaque on each side and grades any narrowing, typically as less than 50 percent, 50 to 69 percent, 70 percent or more, near occlusion or occluded. Mild plaque is common with age and is usually managed with medicines and lifestyle changes. Severe narrowing in someone who has had symptoms may need referral to a vascular or neurology specialist, and further imaging such as CT angiography or MRI.

What to expect at KDRC
Register with your doctor's request and any CT or MRI reports. No fasting is needed. Remove necklaces and wear a top with an open neck. You lie on your back with your head turned slightly away from the side being scanned, and the radiologist moves the probe with gel along each side of your neck from the collarbone to the jaw. You may hear the whooshing sound of blood flow when Doppler is used. Keep still and avoid talking or swallowing while measurements are taken. The study usually takes 20 to 30 minutes and is painless. Our team tells you when to collect the report for your doctor.

Do I need to fast for a carotid Doppler?
No. Eat, drink and take your medicines as usual. Wear an open-necked top and leave necklaces and earrings at home.
What is carotid stenosis?
Carotid stenosis is narrowing of a neck artery, usually from fatty plaque. Severe narrowing increases the risk of stroke, especially in people who have already had a stroke or mini-stroke. The Doppler measures how severe the narrowing is.
Is a carotid Doppler the same as a neck ultrasound?
No. A neck or thyroid ultrasound looks at the thyroid, lymph nodes and lumps. A carotid Doppler looks specifically at the arteries and blood flow to the brain.
Can a carotid Doppler predict a stroke?
It shows one important risk factor, narrowing of the neck arteries. Stroke risk also depends on blood pressure, heart rhythm, diabetes, cholesterol and smoking, which your doctor considers together with the Doppler result.
