What is a penile Doppler?
A penile Doppler (penile duplex ultrasound) examines the blood vessels and tissues of the penis. After a baseline scan, a small dose of a medicine that relaxes the blood vessels is usually injected into the side of the penis to produce an erection. The radiologist then measures the peak systolic velocity in the cavernosal arteries (how well blood flows in) and the end-diastolic velocity (whether blood leaks out too quickly) over several minutes. The scan also shows scarring or plaques in the penile tissue.
Why do doctors order a penile Doppler?
- Erectile dysfunction that has not responded to tablets, or where the doctor wants to know the cause.
- Peyronie's disease: curvature or a lump in the penis, to locate and measure plaques.
- Injury to the penis or pelvis.
- Planning treatment such as surgery.
The test is requested by a urologist or andrologist, who decides whether the injection is appropriate for you.
How much does a penile Doppler cost in Pokhara?

Understanding your results
A peak systolic velocity above about 30 to 35 cm/s after injection generally indicates adequate arterial inflow; below about 25 cm/s suggests arterial insufficiency. A persistently high end-diastolic velocity (above about 5 cm/s) with good inflow suggests a venous leak. Anxiety can reduce the response to the injection, which is why results are always interpreted by your urologist together with your history and examination. The report also describes any plaque, its size and calcification.

What to expect at KDRC
Register with your urologist's request and a list of your medicines, including any blood thinners. The procedure is explained and you give consent. In a private room you lie on your back and the penis is scanned with gel at rest. The injection is given with a very fine needle into the side of the shaft and is usually only briefly uncomfortable. Over the next 5 to 20 minutes, as the erection develops, the radiologist takes measurements at intervals. The whole visit usually takes 30 to 45 minutes. Before you leave, staff check that the erection is subsiding. If an erection lasts more than four hours after the test, it is an emergency (priapism): contact KDRC or go to the nearest emergency department immediately.
